All information must be entered in one session. The data will not be saved if you close the form. Housing Information

Your answers will help determine if the student meets eligibility requirements for services under the McKinney-Vento Act.

Housing and Educational Rights

Students without fixed, regular, and adequate nighttime residences have the following rights:

1. Immediate enrollment in the school they last attended or the local school where they are currently staying even if they do not have all of the documents normally required at the time of enrollment without fear of being separated or treated differently due to their housing situations;

2. Transportation to the school of residency/origin for the regular school day;

3. Access to free meals, Title I, and other educational programs, and transportation to extra-curricular activities to the same extent that is offered to other students

 

All students entering New Jersey public schools must identify the languages that are used to communicate at home. Please identify all language(s) that are spoken in the household in order of usage.

 

The Gloucester County Institute of Technology Food and Nutrition Services Program participates in the National School Lunch Program, which is a federally funded program that assists in providing nutritionally balanced, low-cost, or free meals to students each day. We strongly encourage a free or reduced lunch application to be filled if you believe you qualify. The free and reduced lunch application is used in countless federal programs throughout the year that can benefit families.  Please return all completed forms to the cafeteria staff or the main office. You can submit the application at any time during the current school year.

One form is needed per family per school district. GCIT is unique in that we are our own school district. 
If there are other siblings at other school districts, BOTH school districts must receive an application.

The lunch application must be fully completed (be sure to include everyone in your household and all income). Parents/guardians are highly encouraged to fill out the applications as soon as possible.

IMPORTANT NOTE: For returning families, your current status will stay into effect until September 30th, this is to give you time to fill out a new application. After September 30th any student with a free or reduced status from the previous year without a new application filled out for the 2024-2025 school year will revert to paid status.

 

Note: If your child does not have health insurance and you selected "No" above, we may release your name & address to the NJ FamilyCare Program to contact you about health insurance.

Medications shall be administered in school only with a written order from a primary care provider, along with a signature from the parent/guardian and supervised by the school nurse. The medication order needs to note the name of the drug, its dosage, the interval at which it is to be given, and the diagnosis or reason for its use. All medication must be labeled and in the original pharmacy container. The medication must be brought to the school by a parent or guardian. Medication orders are effective September 1st through the end of the school year.

 

A PHYSICIAN MEDICATION ORDER FORM MUST BE COMPLETED AND RETURNED IN ORDER FOR MEDICATION TO BE ADMINISTERED. 

 

District Requirements Regrading Orthopedic Devices 
Orthopedic devices including but not limited to crutches, wheelchairs, canes, and braces may be needed to help with mobility impairment. Any student entering school with an orthopedic device must have a doctor's note listing the student's name, device, and required length of time needed. Any student entering school without the required doctor's note may be sent home.

 
SNAP Health Portal 
GCIT (Gloucester County Institute of Technology) uses the SNAP Health Portal to better connect you with your child's school health and wellness information. This cloud-based system provides an easy and secure way for you to update and review important school health information and medical documents. It is HIPPA (Health Insurance Portability and Accountability) and FERPA protected program. This is the health office's primary method of communication. 

Please review your Emergency Contacts for your child in your SNAP Health Portal. If you have any changes to your child’s emergency contact information please use the link above under PARENT/GUARDIAN INFORMATION to update your child's emergency contact information.

You may electronically sign permission in the SNAP Health Portal, as we have standing orders from our school's physician to administer. Go to www.studentehr.com and select medication on the left-hand side and then OTCs on the right-hand side. Whatever you want the nurses to be able to give your child, please click "opt-in" and then click "done." This is a cloud-based system and will appear instantly. You do not have to bring in OTC medication. You do not have to have a physician complete the medication form unless the dosage is different from the standard dosing. It is strongly recommended that ALL parents electronically sign the consent form for Tylenol/Ibuprofen, so that medication may be available to treat your student.

 

Harassment, intimidation or bullying" means any gesture, any written, verbal or physical act, or any electronic communication, whether it be a single incident or a series of incidents, that is reasonably perceived as being motivated either by any actual or perceived characteristic, such as race, color, religion, ancestry, national origin, gender, sexual orientation, gender identity and expression, or a mental, physical or sensory disability, or by any other distinguishing characteristic, that takes place on school property, at any school-sponsored function, on a school bus, or off school grounds, as provided for in section 16 of P.L. 2010, chapter 122, that substantially disrupts or interferes with the orderly operation of the school or the rights of other students.

 

 

HIB Policy
 

GCIT is home to School Based Youth Services (SBYS), a statewide, grant-funded program through the New Jersey Department of Children and Families, Division of Family and Community Partnerships. The mission of SBYS is to provide an array of services and experiences that assist in keeping our youth safe and healthy while successfully transitioning into adulthood. 

The program offers a comprehensive set of services to teenagers on a one-stop shopping basis in an accessible location within GCIT. These services include, but are not limited to: individual, family, and group counseling, substance abuse education and counseling, recreational activities, career/employment guidance, leadership opportunities and referrals for preventive, primary and mental health care. All services provided by SBYS are both free and confidential.

Consent is required for students to participate in any SBYS services or endeavors, which include recreational trips such as hiking, sporting events and cultural activities, field trips, SBYS Red Shirt leadership opportunities, after-school activities, the services listed above and much more.

When you consent to your child’s participation in the NJ SBYS you are committing to your child’s ultimate goal of graduating high school. The SBYS Program is available in 67 high schools and it is important that we continuously ensure the programs are achieving its goal. As a result, each program is required to use the following two tools to determine their impact. The two tools contain less than 15 questions related to your child’s thoughts about the program and/or their role in achieving their high school graduation goal and supports they may need to get there.

The NJ SBYS High School Impact Evaluation will be provided anonymously to students that participate in a program activity during the months of October and March.  The Self-Efficacy tool will be provided to students during their first or second meeting with a SBYS counselor.  When reports are produced individual students will not be mentioned.

Students are not required to complete the evaluation, this is truly voluntary. They also have the right to discontinue participating at any point.

GCIT expects all students to follow the code of conduct. A copy of the Parent/Student Handbook is available below. The Handbook outlines the programs, services, student code of conduct, and pupil-parent rights.

Please note, the Parent/Student Handbook for SY2024-25 is still being updated, and will be released in September. 

This is a request for your permission to take and use photographs and videos of your child while enrolled in our school. 

The Gloucester County Institute of Technology would like to highlight our students and educational programs using photographs and videos. The photographs may be used in the classrooms, school newsletters, brochures, newspapers, and for the school's web presence. The video may be used on the Gloucester County Educational Network and/or broadcast in our school and for the school's web presence.

The "No Child Left Behind Act" established by our Federal Government provides that any secondary level school that received federal funds must release a student's name, address, and telephone number upon request to military recruiters and to institutions of higher education. 

This is a request for your permission to share your child's information with military recruiters and institutions of higher education while enrolled in our program.
