Take Your Child to Work Day Form '26

Dear Parents/Guardians,

“Take Your Child to Work Day” will take place on Thursday, April 23, 2026.

If your child plans to participate, please review the requirements below:

By Friday, April 17, 2026:

• Complete THIS FORM to notify the school of your child’s absence.
• Submission of the form will serve as notification; a phone call is not required.

By Monday, April 27, 2026:

• Submit a letter from your employer on company letterhead confirming your child’s participation.
• The letter must include appropriate contact information.

Please note that both the completed form and employer letter are required for the absence to be marked as excused. If either item is not received, the absence will be recorded as unexcused.
